Job Title/Position

LPN’s and RN’s (Full + Part Time) 

Job Description

Deer Valley Home Health Care Services is looking to hire LPN’s and RN’s to provide skilled nursing care to patients in their homes.  
Nurses primarily work one-on-one with clients unassisted in their homes, with access to clinical nurse supervisor 24/7 via phone. The nurse follows the care plan in the home and reports any changes in client condition to clinical nursing supervisor and doctor. While on the job, the nurse is required to complete agency documentation of nursing assessments and completed tasks. Nurse must be able to adequately respond to emergency situations and perform life saving measures if the need arises.

Job Title/Position

Direct Support Professional – DSP (full & part-time)

Job Description

This position provides daily assistance and supports for individuals with disabilities living in their home. A DSP will encourage clients to become independent lives, while being integrated into their communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Reviews and implements activities related to Individual Support Plans (ISPs) and works towards plan outcomes and objectives. Performs all required documentation.
  • Reviews all organizational communications each shift.
  • Provides education, assistance, mentoring and support to individuals. Supports provided may include bathing, personal hygiene activities, dressing, cooking according to dietary guidelines, maintaining a safe home environment, cleaning, laundry, transportation, etc.
  • Provides assistance, education and modeling behaviors when supporting individuals during community outings and social opportunities.
  • Administers medications and provides medical care and support according to physician orders and RN delegations. Communicates changes in functioning, health status or behaviors to support team.
  • Accompanies individuals to medical appointments, communicates physician orders to the team and completes necessary documentation.
  • Provides positive behavioral interventions as outlined in ISP or behavior development plan.
  • Attends all required staff meetings and opportunities for ongoing education and training.

Position Qualifications

• Must have high school diploma or equivalent.
• Must have Med Level I (LIMA), First Aid/CPR, Insulin Certification and MANDT training is preferred but not required before hire.
• Covid Vaccination Card
• Experience providing direct supports to individuals with disabilities preferred.
• Must have basic computer skills and be able to work with a variety of computer systems.
• Must have reliable vehicle and maintain current driver’s license and proof of insurance.
• This position requires driving personal or company vehicle. Transports individuals to appointments, community activities, job sites and other activities/programs.
• This position may require DSP to work - day, evening, night and/or weekend hours depending on the needs of the organization. Overtime hours may be required depending on schedule and organizational need.
• Two references required.

Education/License

Experience

Job Title/Position

Human Resources Assistant (full Time)

Responsibilities

  • Performs customer service functions by answering employee requests and questions.
  • Assist with new hire orientation when needed
  • Assist with new hire training( in-service, CPR, CNA, and other required training) and development efforts. Create flyers and correspondences for staff members concerning training.
  • Assist with EDL and FCSR verification every three months.
  • File papers and documents into appropriate employee files, one day a week or as needed
  • Maintain and create the monthly payroll calendar
  • Make photocopies, fax documents and performs other clerical functions.
  • Make sure office supplies are stocked
  • Performs other related duties as required and assigned
