Ms. Kim Hamm
Intake Coordinator
Ms. Kim Hamm plays a vital role at Deer Valley Home Health by overseeing the intake and admissions process for new clients. She is responsible for gathering admissions information and completing the initial registration for individuals seeking services with our organization. Kim manages client communications through phone, email, and other interactions while ensuring all necessary documentation is completed accurately and efficiently so clients can be approved for in-home health care services.
In addition to coordinating admissions, Kim collects and records detailed information for prospective clients and families, helping ensure a smooth and timely start to care. She also provides guidance and assistance to individuals applying for Medicaid in order to access in-home health care services, helping families navigate what can often be a complex process.
Kim brings over 25 years of experience in the healthcare industry and has proudly served Deer Valley Home Health for more than 14 years. During her tenure, she has held several important roles including Assistant Community Liaison, Risk Manager, CDS Risk Manager, and DMH Coordinator. Her extensive knowledge, dedication to client care, and commitment to supporting families make her an invaluable member of the Deer Valley team.

Ms. Chasatie Fisher

Ms. Fisher has been with Deer Valley for 3.5 years and prior to joining Deer Valley Home Health Services, she was a hospice intake coordinator. Ms. Fisher has excellent communication and customer service skills and is very thorough in the onboarding process.
She is responsible for completing the referral process to verify the Consumer Directed Services program eligibility. She also schedules assessments, trains the consumer as necessary, completes the application process for each attendant of the consumer, completes the FCSR, criminal back grounds, and E-Verify. Ms. Fisher maintains consistent communication with all CDS program participants.
